New Patient Registration

Please ask at reception to request an application form or download, print off, sign, complete, and call into the Surgery or email (as long as application has been signed) this Application Form and New Patient Questionaire. If you live within our Practice area your application will be processed as soon as possible or you can contact the surgery to confirm your application success after 7 days from submitting the application.

We require some form of ID and proof of address and the person wishing to register must present the documentation in person and forms of evidence required are: 

  1. Photographic ID (Passport or Driving Licence) or birth certificate or a letter from DWP and;
  2. One of the following documents as proof of address being less than 6 months old; 

               - Bank Statement

               - Council Tax Bill

               - Utility Bill i.e. Gas/Electric/Water

               - Tenancy agreement 

Please be aware that if married, you will still have to produce evidence for each person. 

If registering any children aged 5 and under, you will need to bring in their ‘Red Book/Child Health Record’ or a complete copy of previous immunisations (you can obtain a copy of this from your previous G.P Practice) before the registration can be processed.

Change of Personal Details

If you change your name, address or telephone number please notify the Surgery as soon as possible including details of dependants.
