HOW THE PRACTICE OPERATES

To Register
Please ask at Reception to request an application form or bring your current medical card. If you live within our Practice area your application will be processed within 48 hours, or you will be informed by letter should there be a problem.

Registered patients aged 16 - 74 who have not been seen for three years are invited to see the Practice Nurse for a routine health check. We also offer routine health checks to patients aged over 75 years who have not been seen in the previous 12 months.

Area Covered
Wrexham Town Centre and most surrounding villages. Please ask at reception. An area map is available.

Repeat Prescriptions
You can call into the Surgery and drop off your request slip, post it to the Surgery or make arrangements with your local chemist. Telephone requests will only be accepted from the over 70's or the housebound, after 11.00 am. Please allow 48 hours Chemist for home delivery.

Home Visits
Visits are for those too ill to go out or who are housebound. Please phone before 10.30 am if possible.

Disabled Access
Disabled access is available to all areas of the Surgery - please ask if you have a specific difficulty.

Change of Personal Details
If you change your name, address or telephone number please notify the Surgery as soon as possible including details of dependants.

To help us to continue to provide a good service to you - we would ask you to cancel appointments you are unable to keep or no longer require. (A significant number of wasted appointments - means all patients wait longer for an appointment) - please help by informing us.

Patients that are violet or abusive to Practice staff or other persons on the Practice premises - will be removed from the Practice list and services will be provided by the Local Health Board, under the Safe Haven Scheme.

Access To Medical Records
Patients may access their own medical records, written and electronic, by a written request to the Practice Manager. Records may not be moved from practice premises and a charge may be made for copies.

Other health care professionals have access to the records of patients in their care.

Access to people outside the practice team is only ever given with the patient's express written permission.
